Incident management is how a facility reports and learns from what goes wrong in NextHealth Flow. Staff report falls, medication errors, needle-stick injuries, near misses and more, each with the DOH level of harm and a sentinel event flag. Maternal deaths are reviewed in the national MPDSR format and perinatal and infant deaths have their own review. Investigators and the committee review each case, with root cause analysis and CAPA actions tracked to closure.

Incident reporting fails when it is slow to file and nobody follows it up. Flow Incidents makes reporting quick and keeps every case moving until its actions are done.

Built to the DOH requirements

Every report records the DOH level of harm beside the facility's own scale and whether it was a sentinel event. Maternal deaths are reviewed section by section in the national MPDSR format, and perinatal and infant deaths have their own review. For a month with no maternal or infant deaths, Flow sends DOH's monthly zero report through its gateway, on a schedule.

Investigated, not just filed

Each incident is assigned to investigators and reviewed by the incident committee, who approve or return it. Root cause analysis records the cause and contributing factors, the review JCI expects for a sentinel event.

Closed by action

CAPA actions have an owner and a due date, the owner is told by email, and every status change is kept in the incident's history.

Flow sends DOH's monthly zero report, which confirms a month with no maternal or infant deaths, through the DOH gateway, using your facility's own DOH credentials. Each incident report carries the DOH level of harm and sentinel event flag the DOH requirements ask for.

Falls, medication errors, needle-stick injuries, near misses, device failure, equipment damage, theft, fire, abuse or harassment, documentation deficits and others, plus maternal mortality and perinatal and infant death reviews.

A report can be flagged as a sentinel event, investigated with root cause analysis and closed through CAPA actions with owners and due dates.

Flow emails the people responsible when an incident is reported, when an investigator is assigned and when a CAPA action is given to someone.
